29 CFR 1910.134(f)(2): Employees using tight-fitting facepiece respirators were not fit tested prior to initial use of the respirator or annually refitted: Fairhaven Christian Retirement Center located at 3470 N. Alpine Road, Rockford, Illinois: On or about November 18, 2020 through December 29, 2020, the employer did not provide a fit test to all employees required to wear respirators. The employer required employees to wear KN95 filtering facepiece respirators throughout their shifts while employees were performing duties such as administering medication to residents, orally or intravenously, assisting with bathing, feeding, or dressing, on suspected or confirmed positive COVID-19 residents. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
