29 CFR 1910.134(e)(3)(i): The employer did not provide a follow-up medical examination for employees who gave a positive response to any question among questions 1 through 8 in Section 2, Part A of Appendix C, or whose initial medical evaluation demonstrated a need for a follow-up medical examination: Northcrest Acres Nursing & Rehabilitation Center, LLC DBA Northcrest Rehab and Nursing Center located at 240 Northcrest Drive, Napoleon, OH 43545: On or about October 31, 2021 and at times thereafter, the employer did not provide follow-up medical examinations to each employee who gave a positive response to any question 1 through 8 in Section 2, Part A of Appendix C (OSHA Respirator Medical Evaluation Questionnaire). The employer required all employees to wear respirators throughout their work shift to help prevent the spread of COVID-19 and while exposed to suspected or confirmed positive COVID-19 residents.
