29 CFR 1910.134(m)(2)(i): The employer did not establish a record of the qualitative and quantitative fit tests administered to an employee which included the information required by 29 CFR 1910.134(m)(2)(i)(A) through (m)(2)(i)(E): a) Michael E. DeBakey VA Medical Center located at 2002 Holcombe Blvd., Houston, TX 77030 On or about July 8, 2020, the employer did not provide respirator fit test records, which included type of fit test performed; specific make, model, style, and size of respirator tested; and the pass/fail results for qualitative or fit factor results for quantitative tests. The employer required employees to wear N95 filtering facepiece respirators to protect against the SARS-CoV-2 virus while providing care to suspected and confirmed positive COVID-19 patients.
