29 CFR 1910.134(k)(1): The employer did not provide effective training that covered the required elements in 1910.134(k)(1)(i) - (vii): a) Healthcare Services Group, Inc. located at 10935 S Halsted St, Chicago, IL, 60628: On or about July 30, 2020, the employer did not provide employees with effective training on the effects of improper fit, limitations, maintenance and care, medical signs and symptoms preventing effective use, and seal check of filtering facepiece respirators. The employer required employees to wear KN95 filtering facepiece respirators to protect against the SARS-CoV-2 virus while providing services to suspected and confirmed positive COVID-19 residents.
