Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to potential injury from crushing caused by ascending and descending elevator cars and/or falling down unprotected elevator shafts: Elevator manlifts, ground floor - a guard or gate was not provided to keep employees from underneath the descending manlift. a) The gate on the manlift was not interlocked requiring the gate to be shut when the manlift was in operation. b) The gates on the second floor and bin floor were not interlocked to prevent the manlift from moving until the gates were closed. Feed mill manlift, ground floor - a guard or gate was not provided to keep employees from underneath the descending manlift. a) An interlock on the south door was not working, allowing the door to be open when the manlift was in operation. b) No gate was provided on the bin floor to guard the open shaftway. Among other methods, one feasible and acceptable abatement method to correct these hazards is to construct a barrier on the ground floor around the hoistways, so employees cannot be underneath the descending manlifts; and to install interlocks on the doors of the elevator manlifts and on the gates to prevent the manlift from moving before the gates are closed.
