OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish the employees employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm in that employees were exposed to crushing hazards: a) Warehouse aisle 406: Employees were exposed to racks storing products three tiers high that had a dented end cap, on or about 8/12/15. b) Warehouse aisle 348: Employees were exposed to racks storing products three tiers high that had a dented end cap, on or about 8/12/15. c) Warehouse aisle 289: Employees were exposed to racks storing products three tiers high that had an end cap with an unattached cross brace, on or about 8/12/15. d) Warehouse aisle 231: Employees were exposed to racks storing products three tiers high that had a dented end cap, on or about 8/12/15. e) Warehouse between slots 002 and 003: Employees were exposed to racks storing products three tiers high that had a twisted leg on level d, on or about 8/12/15. f) Warehouse between slots 052 and 053: Employees were exposed to racks storing products three tiers high that had a twisted leg on level d, on or about 8/12/15. g) Warehouse between slots 014 and 015: Employees were exposed to racks storing products three tiers high that had a twisted leg on level c, on or about 8/12/15. h) Warehouse between slots 129 and 130: Employees were exposed to racks storing products three tiers high that had a twisted leg on level d, on or about 8/12/15. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
