29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): On or about November 11, 2015, the employer did not ensure point of operation guarding was in place to protect employees from having any part of their body in the danger zone of the north Amada press brake #72 during operation. Employees were exposed to hazards associated with amputations and being caught-in the machinery. Temtrol Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(3)(ii), which was contained in OSHA inspection number 933984, citation number 1, item number 2 and was affirmed as a final order on February 4, 2014, with a final abatement date of May 30, 2014, with respect to a workplace located at 15 E. Oklahoma Ave., Okarche, OK 73762.
