29 CFR 1910.119(j)(6)(ii): Appropriate checks and inspections were not performed to assure that equipment was installed properly and consistent with design specifications and the manufacturer's instructions. On or about April 26, 2016 and there prior, the employer did not conduct the following: a. Appropriate checks and inspections to ensure the piping/weld was proper and consistent with design specifications, regarding a welded end-cap on a 16 inch suction pipe that released ammonia in the DSI workroom. b. The employer did not conduct inspection checks such as radiographic or ultrasonic testing to ensure defects in welded surfaces were identified.
