29 CFR 1910.22(b)(1): Where mechanical handling equipment was used, aisles and passageways had obstructions across or in them that could create a hazard. (a) IQF room - On or about June 28, 2016, employees were exposed to crushing hazards when operating pallet jacks to enter doorways equipped with metal and/or concrete bollards within the passageway. In the Alternative: Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing hazard(s): (a) IQF room - On or about June 28, 2016, employees were exposed to crushing hazards when operating pallet jacks to enter doorways equipped with metal and/or concrete bollards within the passageway.
