29 CFR 1926.651(j)(1): Adequate protection shall be provided to protect employees from loose rock or soil that could pose a hazard by falling or rolling from an excavation face. Such protection shall consist of scaling to remove loose material; installation of protective barricades at intervals as necessary on the face to stop and contain falling material; or other means that provide equivalent protection. 50 Cedar Street, Somerville, MA 02143 On or about June 12, 2017, employees were exposed to loose soil while performing pipe work in a trench 15 feet in depth where the protective system did not extend to the employees' work area. On or about June 12, 2017, an employee was struck by a piece of soil (clay) while in the trench and sustained a serious back injury requiring a response and rescue by the Somerville Fire Department.
