29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: The employer failed to prevent exposure to unintended energization of equipment. On or about 08/03/2017, Aim to Amaze Outdoor Services Inc., located at 4027 Gravois Road, House Springs, MO, had not developed or trained on an Energy Control Program for equipment that contained multiple forms of hazardous energy, to include but not limited to hydraulic, electric, and rotational energy. Employees serviced and maintained company equipment to include, but not limited to, the Bandit 200 diesel wood chipper.
