29 CFR 1910.28(b)(3)(i): Holes. The employer did not ensure each employee was protected from falling through any hole (including skylights) that was 4 feet (1.2 m) or more above a lower lever by one or more of the following: a) Lumber/Mill Yard (Debarker/Metal Detector's Catwalk Area): On or about August 01, 2017, and at times prior thereto the employer exposed employees to potential fall hazards in that exposed employees were required to use a catwalk that have holes that were 10 feet (3.048 m) or more above a lower level and were not protected by covers or guardrail systems. b) Lumber/Mill Yard (Head Saw Area): On or about August 01, 2017, and at times prior thereto the employer exposed employees to potential fall hazards in that exposed employees were required to perform work and "flip wood" at or near holes that were 5 feet (1.524 m) or more above a lower level and were not protected by covers or guardrail systems.
