29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means: The employer is failing to prevent employee exposure to contact with electricity. The condition was most recently observed on or about September 06, 2017, at 312 Cookshire Lane, Chesterfield, Missouri. The employer did not ensure that overhead 34.5 kilovolt outer phase powerlines were guarded by insulation or other means. Employees used electrically conductive, aluminum poles during concrete finishing work. The poles measured approximately 23 feet in combined length. The overhead powerline measured approximately 21 feet 9 inches from the work surface. An employee suffered fatal injuries as a result of this condition.
