OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees. (a) Coke Battery, Coal Bunker/Larry Car: On or before December 3, 2017, the employer did not require the energy control procedure for the coal charging car (Larry Car) to be utilized to prevent crushed-by, crushed-between, electrical shock, and arcing hazards when unjamming coal stuck in the bunker gate area. (b) Coke Battery, Coal Bunker/Larry Car: On or before December 3, 2017, the employer did not require energy control procedure for the coal charging car (Larry Car) to be utilized to prevent crushed-by, crushed-between, electrical shock, and arcing hazards when the need arose to retrieve the air lance from the coal hopper.
