29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees. On or about March 21, 2019, the employer did not ensure that employees who performed maintenance and/or service work on machinery and/or equipment such as the New Holland Skid Steer, model LS190, serial number N5M418227, exposing employees to struck-by and crush-by hazards. The manufacturer's lockout device was not engaged at the time of the incident.
