29 CFR 1910.147(d)(5)(i): All potentially hazardous stored or residual energy was not relieved, disconnected, restrained or otherwise rendered safe after the application of lockout or tagout devices to energy isolating devices: (a) West-facing side of I-Guide Platform: On or before June 13, 2019, and at times prior, the employer exposed employees to struck-by hazards, in that the employer did not require employees to relieve stored or residual energy in a chain and sprocket assembly before trying to remove it for repair. NOTE: ENERGY SHOULD BE AT ZERO STATE.
