The employer is failing to ensure that all nameplates and markings are in place and maintained in a legible condition. At 6190 Mid Rivers Mall Drive in St. Peters, MO 63304 - On or about 07/23/2019, employees were operating a "Crown" (Model No. RR5725-45; Serial No. 1A422917) powered industrial truck that had the capacity rating scratched off the capacity nameplate and was not maintained in a legible condition, creating the condition that the employees could overload the truck causing it to tip over on the operator or other employees in the area.
