29 CFR 1910.421(d)(5): Planning of a diving operation did not include an assessment of the safety and health aspects of the diving equipment and systems: On or about November 12, 2019, a fatality of an Underwater Construction Corporation (UCC) in-water tender occurred during the initial set-up of a diving operation at the Exelon Nuclear LaSalle Station in Marsellies, IL. The employee was exposed to hazards due to the operation of a pump which may cause differential pressure (Delta P) changes along with other hazardous underwater conditions. The employer did not ensure that the planning of diving operations included a proper assessment of the diving equipment and systems necessary to prevent or correct hazards that may arise during underwater activities. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
