29 CFR 1910.178(q)(7): Industrial trucks were not examined, at least daily or before each shift if used on a round-the clock basis, before being placed into service: The employer is failing to protect employees from struck-by and crushed-by hazards. This was most recently documented on or about February 18, 2020 at the facility located at 1915 East Kansas City Road, Olathe, Kansas. The following Kalmar hostler trucks were being operated at the facility to move trailers around the dock lot and a maintenance building forklift were not inspected daily or prior to each shift: 1. Employee was operating hostler truck #129369 without identifying no working horn, rear brakes operating intermittently, and bottom outside right side round mirror was missing. 2. Employee operating hostler truck #135632 without identifying no brake lights and no left / right turn signal lights. 3. Employee operating Clark Forklift #500523 without identifying lack of labeling for the control levers pertaining how the forklift operates. 4. Employees operating hostler truck #102613 without identifying bottom outside left side round mirror being missing.
