29 CFR 1926.1204(c)(1): The employer did not develop and implement the means, procedures, and practices necessary for safe permit space entry operations, including specifying acceptable entry conditions: On or about July 9, 2020, at or near the intersection of Mitchell St. and W. Barkley St. in Sour Lake, TX, the employer did not implement any procedures for safe permit space entry, exposing employees to toxic sewer gases and engulfment hazards after entering a sanitary sewer manhole.
