29 CFR 1904.39(a)(1): Within eight (8) hours after the death of an employee as a result of a work-related incident, the employer did not report the fatality to the Occupational Safety and Health Administration (OSHA), U.S. Department of Labor: a) Luling Care Center located at 501 W Austin Street, Luling, TX 78648: On or about July 17, 2020, an employee died of COVID-19. The employer did not report the work-related fatality to OSHA within 8 hours. The employer reported the death to OSHA on August 12, 2020 during a phone discussion regarding a staff member death from COVID-19 at a sister facility within the same city.
