29 CFR 1904.40(a):The employer did not provide an authorized government representative the records within the four business hours. a) Norridge Gardens, LLC located at 7001 W. Cullom Avenue, Norridge, IL 60706: On August 24, 2020, Norridge Gardens, LLC, upon request, did not provide a copy of the OSHA Form 300, Log of Work-Related Injuries and Illnesses for calendar year 2020, within the four business hours. The OSHA 300 logs were not provided until December 14, 2020, after repeated requests. No abatement documentation or certification required for this item.
