29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control: a) Optimized Sharp Chain Line: On or about December 21, 2022, the employer exposed employees to crushed-by hazards in that the employer did not train employees in the recognition of the types of energy associated with residual pressure in hydraulic accumulators and the means necessary for energy isolation and control of hydraulic energy to prevent the unexpected movement of modules when servicing and maintaining the equipment.
