29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: On or about June 20th, 2023, employees were not trained to recognize and control struck-by hazards due to energy sources where repair operations on the CEMCO 150 cement mixer were manually attempted on the overhead auger shaft without isolating the energy sources.
