Outside of Terminal C Gate C26 at Logan International Airport on 1 Harborside Drive, Boston, MA 02128, on or about August 29, 2023, an employee operated a Komatsu BX Forklift (Model Number: FG25TLP-12; SN: 515775A) with the forks and mast raised where the employer did not ensure that the forks were only raised as far as necessary.
