The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head. a) On or about May 21, 2024 at the above address the employees were exposed to fall hazards of approximately of 17 feet ground to eave while performing residential roofing work while not having the attachment point of the safety harness attached at the center of the employees back.
