Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by and crushed-by hazards from the special purpose personnel elevator: Employees working on the ground floor level in the Head House of Elevator A were exposed to injury in that the landing opening for the special purpose personnel elevator was not protected by a barricade preventing entry into the landing area when the elevator is traveling downward. Among other methods, one feasible and acceptable method to correct this hazard is: Enclose the landing area with a 7 foot high door with the controls for the manlift outside of the enclosed area in accordance with ASME A17.1-1996, American Society of Mechanical Engineers, in Safety Code for Elevators and Escalators, Rule 1500.6a Hoistway Doors and Gates, which requires that the full width of each landing opening be protected to its full height, or 7 feet, whichever is less, by doors or gates. 29 CFR 1903.19(c)(2) abatement of the above violation was verified at the time of inspection. No certification is required.
