29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: a/ Inside of the Reclaim Department, on or about 03/19/15, the 480 volt electrical disconnect belonging to the Tumbler Rotary Valve, unknown make, model, or serial number, was not turned off prior to an employee reaching in to clear a jam. This employee suffered from multiple amputations of his fingers.
