29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: a) Production area: The employer did not provide training to authorized employees that cleaned, serviced or maintained equipment, such as, but not limited to, a Jagenberg folding machine, a Hydromat Three Knife Trimmer model SL III M-900 serial number 1822/900/98 and Thomson Die Cutting machines, on or about 7/13/15. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
