29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: The employer is failing to protect employees from electric shock hazards while utilizing electric extension cords during construction activities on site. The employer is failing to ensure that the cords they are utilizing are properly grounded. This was most recently documented at the corner of 204th St. and Maple St., Elkhorn, Nebraska on or about June 21th, 2017 when the employees were using an extension cord on site that is missing the grounding prong.
