29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladder(s) were not used at an angle such that the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder: (a) On or about January 8, 2018 at the jobsite on the south side of the new home; where employees were accessing the second story roof with an extension ladder that was not positioned so that the base of the ladder was one-quarter of the working length of the ladder from the wall.
