29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment: a) On or about December 17, 2018, at the facility, where each authorized employee did not affix a personal lockout device to a group lockout device before working on Screw Conveyor 515 Bin 11. Employees were exposed to caught-in hazards when performing maintenance tasks on Screw Conveyor 515 Bin 11, such as removing obstructions from the screw auger.
