29 CFR 1926.251(a)(1): Rigging equipment for material handling was not inspected prior to use on each shift and as necessary during its use to ensure that it was safe and defective rigging equipment was not removed from service: a) 801 Boxwood Road, GM Assembly Plant, Wilmington, DE: On or about January 11, 2019, the employer failed to ensure rigging equipment used with the Komatsu PC1250 LC and CAT 349 FL Excavators were adequately inspected to identify unsafe conditions and removed from service. Employees were exposed to being struck by wire ropes being used to pull on the structural members of a building being demolished, where the wire rope contained broken strands and no permanently affixed markings.
