29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: a) On or about July 4, 2019, and at times prior thereto, employees were exposed to struck-by hazards while operating a Nissan 50 with an Abaco Hydraulic Jib Boom AFJ-25 attachment without the manufacturer's written approval. b) On or about July 4, 2019, and at times prior thereto, employees were exposed to struck-by hazards while operating a Nissan 30 with an Abaco Hydraulic Jib Boom AFJ-25 attachment without the manufacturer's written approval.
