29 CFR 1910.146(d)(4)(i): Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not ensure that employees used the testing and monitoring equipment properly: On March 26-27, 2024, Employees were exposed to combustible hazards in that the atmosphere was not tested with a 4-gas monitor after the portal cover was removal, and prior to entry into the gun barrel tank.
