29 CFR 1910.133(a)(3): The employer did not ensure that employees wearing prescription lenses while engaged in operations that involve eye hazards wore eye protection that incorporates the prescription in its design or wore eye protection that can be worn over the prescription lenses. On or about 3/21/2016, the employer failed to ensure and/or enforce the use of adequate eye protection for employees wearing prescription lenses. Employees using the company's vertical band saw, drill press machinery or CNC machinery were not protected from exposure to flying chips, sparks or objects.
