29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): A) Nox US, LLC, located at 931 South Springville Ave, Fostoria, OH 44830: On or about July 11, 2017, the employer did not ensure that employees implement energy control procedures on the equipment such as, but not limited to, the hopper's butterfly valves of the PVC Recycle System on Line 1 prior to conducting activities such as, but not limited to, unplugging / cleaning the hopper. The hopper's valves were not isolated from pneumatic energy source prior to engaging in these maintenance activities. Employees were exposed to amputation hazards while all hazardous energy sources were not controlled prior to conducting work. B) Nox US, LLC, located at 931 South Springville Ave, Fostoria, OH 44830: On or about August 4, 2017, the employer did not implement energy control procedures on the equipment such as, but not limited to, the grinder of the PVC Recycle System on Line 1 prior to conducting activities such as, but not limited to, rebuilding the grinder. The grinder was not isolated from the electrical energy source prior to engaging in this maintenance activity. Employees were exposed to amputation hazards while all hazardous energy sources were not controlled prior to conducting work.
