29 CFR 1910.23(b)(4): Ladder rungs, steps, and cleats did not have a minimum clear width of 16 inches (41 cm) for fixed ladders: On or about April 11, 2018, an employee working on top of Bin #4 was exposed to fall hazards when climbing the tapered fixed ladder on the bin roof due to the ladder rungs did not have a minimum width of 16 inches.
