29 CFR 1926.859(g): During demolition, continuing inspections by a competent person were not made as the work progressed to detect hazards resulting from weakened or deteriorated floors, or walls, or loosened material. Employees were also permitted to work where such hazards existed without being corrected by shoring, bracing, or other effective means. On or about January 16, 2019, employees were exposed to a struck by hazard while demolishing an exterior elevator shaft located at 8630 Evins Ave., Cleveland, Ohio 44104. An employee was using a 400 excavator to demolish the shaft and the employer did not ensure that continuing inspections were done by a competent person.
