29 CFR 1926.502(d)(15): Anchorage points used for attachment of personal fall arrest equipment were not independent of any anchorage point being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee: a. On or about March 29, 2019, at the above addressed jobsite, an employee conducting copper roofing operations from a bay window roof, approximately thirty-five (35) feet above the ground, had a lifeline secured to roof anchor plate with three (3) nails total, thereby exposing the employee to fall hazards.
