29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders, as necessary, which would enable each employee to recognize hazards related to ladders and train each employee in the procedures to be followed to minimize these hazards. (a) Peninsula-McCabe United Methodist Church, 2200 Baynard Blvd., Wilmington, DE (church sanctuary) - On or about January 2, 2021, the employer failed to train each employee who used a Werner 32-foot 32FT Type IA Fiberglass D-Rung Extension Ladder (Model D6232-2) set a top a scaffold's platform, resulting in the death of an employee who fell a distance of approximately sixteen feet off this extension ladder to the floor.
