29 CFR 1910.178(o)(1): The employer did not ensure that only stable or safely arranged loads were handled: (a) 98 East Main Street, Leola, Pennsylvania - To reach elevated work, employees used a large in-house made personnel platform (with a rusted patina) on the forks of any one of the following powered industrial trucks: Nissan forklift (model BGF03A45V, chassis number BGF03-920953), UniCarriers forklift (model MUG1F2A35LV, chassis number UG1F2-9L21014), or UniCarriers forklift (model MUG1F2A35LV, chassis number UG1F2-9L21246). This platform had no means for secure attachment to the lifting carriage and forks, which could result in dropping the personnel platform and its occupants, on or about May 8, 2024. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
