29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees: A) Nox US, LLC, located at 931 South Springville Ave, Fostoria, OH 44830: On or about July 11, 2017, the employer did not properly train employees to lockout/tagout equipment such as, but not limited to, the grinder and the hoppers butterfly valves of the PVC Recycle System on Line 1. Employees did not control all hazardous energy sources such as, but not limited to, electrical and pneumatic before employees conducted activities such as, but not limited to, rebuilding the grinder and cleaning the hopper. Employees were exposed to amputation hazards while all hazardous energy sources were not controlled prior to conducting work.
