29 CFR 1910.147(d)(3): All energy isolating devices that are needed to control the energy to the machine or equipment shall be physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s). a) On or about February 6, 2024, and at times prior thereto, employees were exposed to struck-by hazard while working on equipment without isolating device to isolate the machine or equipment from the energy source.
