29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees a) Production area: The employer did not provide locks or tags to employees to secure energy isolating devices when cleaning, servicing or maintaining machines, such as, but not limited to, a Jagenberg folding machine, a Hydromat Three Knife Trimmer model SL III M-900 serial number 1822/900/98 and Thomson Die Cutting machines, on or about 7/13/15. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
