29 CFR 1910.333(b)(2): While an employee was exposed to contact with parts of fixed electric equipment or circuits which were de-energized, the requirements of paragraph (b) of 29 CFR 1910.333 were not followed in the order in which they are presented therein (i.e., paragraph (b)(2)(i) first, then paragraph (b)(2)(ii), etc.): On or about June 26, 2017, at 7000 N Beach St., Fort Worth, TX, procedures were not developed or maintained for locking and tagging out energized circuits exposing employees to electrical shock and electrocution hazards.
