29 CFR 1910.147(c)(5)(ii)(C)(1): Lockout devices were not substantial enough to prevent removal without the use of excessive force or unusual techniques, such as with the use of bolt cutters or other metal cutting tools: On or about October 17, 2017, the employer did not ensure that employees utilize locking devices specifically design to prevent removal of improper application or inadvertently by contact with another object or worker before performing maintenance on the nose landing gear doors of the Airbus A310, exposing employees to struck by and caught in hazards.
