29 CFR 1910.119(j)(5): The employer did not correct deficiencies in equipment that were outside acceptable limits (defined by the process safety information in paragraph (d) of this section) before further use or in a safe and timely manner when necessary means were taken to assure safe operation: Employees engaged in operating, inspecting, testing, and maintaining (preventive maintenance, service, repair) activities at the Central Utility Building (CUB) and in the Ammonia Engine Room were exposed to the hazards of being struck-by over-pressured equipment and toxic ammonia inhalation and burns. The employer fails to fully implement its mechanical integrity procedure, when it did not replace safety relief valves every 5 years or sooner if necessary. The employer did not ensure that the safety pressure relief valves were removed and replaced within 5 years, or develop an alternative replacement interval based on documented in-service relief valve life, or follow the manufacturer's replacement frequency recommendation.
