29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware were not provided by the employer for isolating, securing or blocking of machines or equipment from energy sources: a. On or about September 22, 2018, in the production area, maintenance employees were required to service and maintain equipment that had two or more energy sources, but were only provided one lock for tasks such as, but not limited to, repairing a pump loop or changing a motor. The employees were exposed to the hazard of uncontrolled energy.
