29 CFR 1926.859(e): When pulling over walls or portions thereof, all steel members affected had not been previously cut free. On or about January 16, 2019, employees were exposed to a struck by hazard while demolishing an exterior elevator shaft located at 8630 Evins Ave., Cleveland, Ohio 44104. The elevator shaft was reinforced by steel rails that were attached to all four corners of its interior and the employer did not ensure that they were cut free prior to demolition.
